Please contact our office at 866-722-6363 (toll free) or email info@pacsurgical.com to speak with one of our Bariatric Coordinators to discuss the total cost of the Lap-Band surgery. Our fee covers the following:

- Pre-op consultation with the surgeon
- Pre-op consultation with the nutritionist
- Pre-op labs and testing deemed necessary by the surgeon
- Surgeon fee
- Assistant surgeon fee
- Anesthesiologist fee
- All follow-up appointments with surgeon
- All band “fills” (all band “fills” or “adjustments” are done under fluoroscopy (x-ray) right here in our office)***
- 24 hr. surgeon accessibility

***CHANGE EFFECTIVE JANUARY 1, 2009: Free band fills will be limited to only the first two years following surgery. After two years, there will be a fee charged for each band fill.

Lap-Band surgery for weight loss is covered by some insurance companies; however, Pacific Surgical Weight Loss Center is not contracted with most insurance companies (see below for information regarding Uniform and Aetna plans for State of Washington employees.) There is a fee for the surgeon’s portion of the surgery, however; we will work to get the hospital and anesthesiologist’s bill authorized by your insurance company.

The best way to find out if your particular plan has benefits for weight loss surgery is to call the customer service number on your insurance card. If you like, you may use this handy form when you call; just contact our office with your insurance information at 1-866-722-6363. Once we have determined that your insurance company does cover this type of procedure, we have a process in place for working with you to obtain the necessary pre-approval for surgery. The first step is to compose a letter of medical necessity which we will do for you following your initial consultation with the surgeon.

Pacific Surgical Weight Loss Center, PLLC does not accept DSHS coupons or CHPW for Lap-Band surgery. We also are not contracted with Medicare or Molina Healthcare.

Pacific Surgical Weight Loss Center is now contracted with PEHP for Lap-Band surgery for State of Washington Employees . Please contact your insurance company for information about how to enroll in their bariatric program. Once your criteria is satisfied through that program, you can request to be referred to our clinic for surgery. If you have questions, please call us at 866-722-6363.
